Community Manager – Aged Care & NDIS

Are you a strategic leader with a passion for delivering high-quality, person-centred care? We are seeking an experienced and driven Community Care Manager to lead and grow our clients aged care and NDIS services portfolio.
This is a pivotal leadership role responsible for shaping the future of our clients Community Care programs, ensuring they are responsive, sustainable, and aligned with evolving sector reforms.

About the Role
As the Community Care Manager, you will provide strategic leadership and operational oversight across community aged care and NDIS services. You will play a key role in transitioning their service delivery model to a consumer-directed, market-responsive approach, ensuring services remain relevant, culturally safe, and of the highest quality.
You will lead the design and delivery of innovative, person-centred programs that meet the Aged Care Quality Standards and NDIS Practice Standards, while also driving growth and sustainability.

Key Responsibilities

  • Provide strategic leadership across community aged care and NDIS service delivery
  • Drive the transition to consumer-directed and market-responsive care modelsEnsure compliance with Aged Care Quality Standards and NDIS Practice Standards
  • Lead, mentor, and develop a multidisciplinary team
  • Oversee operational performance, financial sustainability, and service quality
  • Identify and develop new service opportunities to support business growth
  • Expand client reach and strengthen referral pathways
  • Build and maintain strong partnerships with key stakeholders and community networks
  • Lead the development of innovative and culturally appropriate care models

About You

  • Proven leadership experience in aged care, disability services, or community services
  • Strong understanding of the NDIS and aged care sector reforms
  • Demonstrated experience in service growth, program development, and innovation
  • Sound knowledge of compliance frameworks and quality standards
  • Excellent stakeholder engagement and partnership-building skills
  • Financial and operational management experience
  • A commitment to culturally safe and person-centred care
